GENERAL INFORMATION TO THE TECHNICAL TRAININGS

I. BMD DEXA technical training now includes 1 day of basic application training. This is to ensure basic end user

operation of the units.

II. The seats are limited. Please register in time and confirm your seat by sending the PO for the training. Fujifilm reserves the right to cancel the training if less than 5 persons register for the training. Full payment is required 45 days before the training date, otherwise registration will be cancelled.

III. Cancellation Policy:

• Full payment is required 45 days before the training date, otherwise registration will be cancelled

• Cancellation before one month of the training date will be eligible for an 80% refund.

• Cancellation before 2 weeks of training date will be eligible for an 50% refund.

• Cancellation less than 2 weeks of training date or failure to attend (“no show”) will not be eligible for any refund

IV. Mandatory Prerequisites for training attendance

1) FPDs Technical Training:► Networking Knowledge.

2) FCR Technical Training: ► Networking Knowledge.

3) FDR Nano/AI Advanced Technical Training: ► Passing FDR Nano Basic training.

► Passing FPDs Training &/or FCR training. ► Installation of two machines

4) FDR GO +/AI Advanced Technical Training: ► Passing FDR GO+ Basic training.

► Passing FPDs Training &/or FCR training. ► Installation of two machines.

5) Amulet Innovality Advanced Technical Training: ► Passing Innovality Basic training. ► Installation of two machines.

6) FCT Speedia/Supria Basic Technical Training: ► Two years of CT Experience. ► OR 5 years of X-RAY Experience.

7) FCT Speedia Advanced Technical Training: ► Passing FCT Speedia/Supria Basic Training. ► Installation of two machines.

8) Scenaria View Advanced Technical Training: ► Passing FCT Speedia/Supria Basic Training.

►Installation of two machines.

9) FDR Smart X (Floor/ceiling) Basic Technical Training: ► Passing FPDs Training &/or FCR training.

10) FDR Smart X (Floor/ceiling) Advanced Technical Training: ► Passing FDR SMART X Basic Training.

► Installation of two machines.

11) FDX Visionary DR Advanced Technical Training: ►Passing FDX Visionary DR Basic Training.

► Installation of two machines.

12) FDX Visionary C Basic Technical Training: ►Two years of Experience in X-ray systems.

13) MRI E-Synergy ZBO/ZHe Technical Training: ► Two years of Experience in MRI

►MRI Physics Knowledge.

V. If needed, an online test / Interview will be conducted for participants before starting the training.

VI. The certificate will only be issued upon scoring/passing rate of at least 70%.

VII. The following discounts are applicable depending on the total number of seats booked by the company:

VIII. The above discounts are only applicable upon issuing ONE PO for the requested total number of seats. The individual invoice will be shared after every training, and the payment shall be arranged accordingly.

IX. Discount will be waived if the total number of the requested seats is not equal to the total number of participations. Also, more discounts will be applied to the total number of seats exceeded the requested ones as per the table above.
